Minimum Qualifications of Education and Experience

Expected starting salary is $33.45 to $44.66.  Full salary range $33.45 to $55.87.

Bachelor's or Master's degree from an accredited college or university.

Regular and reliable attendance is a necessary component of job/position. Individuals required to use County vehicles and equipment must have a valid driver's license and be free of any major traffic violations for the last three (3) years.

Desirable Qualifications:

Two years of social service-related experience.

Certification from the Permanency Adoption Competency Certificate (PACC) from University of Minnesota or National Training Institute (NTI) Adoption Competency Mental Health or similar

Nature of Work

Under supervision, and within Federal, State and locally developed programs and policies, identifies individual and/or family needs and works towards meeting those needs through County and community support services.  Will provide ongoing outreach and supportive connection to families who have finalized adoptions or Transfers of Permanent Legal and Physical Custody (TPLPC) from Minnesota foster care.

Provides short term assessment and possible case management to individuals and families.  Will include identification and coordination of residential treatment services for children/youth in crisis being served by Child and Family Services Permanency Team.  May provide education in community-based settings.  Works within a multi-disciplinary team setting.  Partnership based collaborative social practice supports family involvement strategies as part of an effort to promote child safety, well-being and permanency, Supports a diverse, respectful and inclusive workplace. 

Examples of Work

  • Completes home visits and utilizes person-centered engagement strategies to assess needs, strengths within individuals and family systems. 
  • Partner with families in the development of case plan goals that leverage family strengths to address barriers or challenges. 
  • Advocacy for children, families, and communities. 
  • Advise clients of possible resources, rights, and responsibilities under programs or services 
  • Refers clients to other programs or agencies and assists with access when appropriate 
  • Works with natural supports and family group decision making 
  • Prepares correspondence reports and social histories and maintains case records in accordance with state, federal and local guidelines
  • Maintains electronic case file documentation in compliance with program requirements.
  • Actively participate in weekly team consultations
  • Organize, lead and support monthly post-adoption support group. 
  • Perform related work as required.

  • Knowledge of emotional and psychological aspects of adoption or other permanency outcomes.

  • Skilled in verbal communication, public speaking, writing and recording of information and data
  • Skill in the use of computers and technology
  • Skills in individual and community advocacy
  • Ability to engage in difficult conversations with families while maintaining compassion

  • Ability to exercise good judgment in recommending possible solutions for individual and family needs and when utilizing conflict management and advocacy skills
