Description:
The Nurse Practitioner is responsible for providing comprehensive and compassionate medical care to hospice patients focusing on symptom management, pain relief, and quality of life. The Nurse Practitioner works collaboratively with the Medical Director and Interdisciplinary Team (IDT) to assess patients, develop care plans, prescribe medications, provide patient and family education, and coordinate care throughout the end-of-life journey.
Education and Training Requirements:
- Master of Science in Nursing from an accredited institution required.
- 3+ years of experience as a Nurse Practitioner, with a strong preference for experience in hospice, palliative care, or a related field.
- Active and unrestricted RN and Nurse practitioner licenses in Minnesota required.
- Certified as a Nurse Practitioner in Adult, Family or Gerontology required.
- Active DEA registration for prescribing medications, including controlled substances required.
